page numbering oddity

Using Docbook x4.1.2 and db164, with jade, when creating a pdf, there are a
couple of strange page numbering behaviors.
First, the ToC skips page numbers. For example, Chapter 1 *and* Chapter 2
both have the page number of "1". The page number in the footer is correct.
Second, the page number for the introduction/preface is listed on the ToC as
"i", which is correct, but on the footer as "3".
I am an absolute newbie to dsssl, so are there any simple fixes for this?
