indexterm + pageno.

If I generate an index for print backend (RTF and TeX) I get
question marks instead of pagenumbers for the indexterm's.
Environment:
- docbook SGML V4.1
- docbook DSSSL stylesheets V1.59
- collateindex.pl
genindex.sgm was created using Norm's suggestion at:
http://nwalsh.com/docbook/dsssl/doc/indexing.html
with the command line:
jade -t sgml -d html/docbook.dsl -V html-index document.sgm
The generated genindex.sgm looks good for me.
What is the solution for this?
