well of course you've already strayed a bit by using xsl:document but anyway...
Can't your template just make the filename, and then for xsl:document
you do
<xsl:variable name="foo">
<xsl:call-template name="work-out-filename"/>
</xsl:variable>
<xsl:document href="{$foo}">
....
and for linking you do
<a>
<xsl:attribute name="href">
<xsl:call-template name="work-out-filename"/>
</xsl:attribute>
....
