[PATCH] Always do locking when accessing streams (bug 15142, bug 14697)

- Subject: [PATCH] Always do locking when accessing streams (bug 15142, bug 14697)
Now that abort no longer calls fflush there is no reason to avoid locking
the stdio streams anywhere.
