Tested (x86_64-linux) and finished now.  A nice side effect of this
patch is that "break main; run" on a threaded program no longer
displays the "[Switching to Thread ...]" message on GNU/Linux; this
was an artifact of adding the TID to inferior_ptid.

Any comments?  Otherwise I'll commit this in a few days.

Vladimir, this patch should allow you to add the initial thread to
the thread list.  The best place is probably linux_nat_attach and
linux_nat_create_inferior.  Then "info threads" will always work
for the Linux native target.  Similarly for remote if it doesn't
already.  I think we should fix this one target at a time, since
changing it in the GDB core is likely to break other threading
targets.
