PATCH: gdbserver README and configury


The README was woefully out of date.  I sat down to update it only to
discover that the new documentation I was writing on how to configure
gdbserver did not actually work.  The changes were all just
Makefile/configure work, so I can just commit them as obvious.  The README I
also consider obvious, now that I've made the code behave.

Committed.  gdbserver now builds almost completely independently of GDB; it
can be configured separately.  We still use "terminal.h" - work of ten
minutes to remove - and the host/target Makefile fragments.

What do people think about removing gdbserver configuration from both the
toplevel configure.tgt and the Makefile fragments, and moving it into a
shell script in the gdbserver directory?  That would reduce a gdbserver
distribution to the gdbserver/ and regformats/ directories, basically, and
clean up a bit of cruft in the process.
