Without the copy of the header.content template in the driver file, all
is well. I have seen this customisation discussed on the list, so it
must work for others. Any ideas?
your style sheet does not have the fo declaration
<?xml version="1.0" encoding="UTF-8"?>
<xsl:stylesheet xmlns:xsl="http://www.w3.org/1999/XSL/Transform"
xmlns:fo="http://www.w3.org/1999/XSL/Format"
