I'm not sure I understand this problem.
Could you clarify which of your output example
is meant to be tables of contents?
When I process your sample file with xsltproc
or Saxon, I get what you want (I think).
It appears from your sample out that
XalanC is losing the section titles.
If so, that's a pretty big bug.
You might try another processor.
