Well, I spent more time searching in the mailing list archive and on google
and couldn't come up with posts that had the same issue.
Most of the posts I could find would get an error message when they tried
to use mkpasswd -d
I don't get an error message but I actually get the first 12 entries of the
domain user list. Same thing when I try the mkpasswd -l on the actual
Primary Domain Controller. Again I don't get an error message but the first
12 users of the ~400 users.

You might want to check the archives on this topic. There was some
discussion of the different domain controller configurations and it's
affect on mkpasswd. You might find some of that discussion useful for
your problem.
