This is currently entirely unspecified.

If mydocument.xml has a stylesheet processing instruction[1], then a system 
might use that.  Otherwise, there's no way to instruct a hypertext system 
to open document X with stylesheet Y.

There is a W3C note[2] that explores the issue further and makes some 
suggestions on how to proceed, but there is nothing normative yet.
