You probably want to be setting the 'checked' attribute to 'checked'
rather than 'true'. The SGML/XHTML/XSLT rules about attributes with
no values (e.g. <input checked />) is that the value is taken as being
the name of the attribute. So use:
<input name="{name}" type="{type}" value="{value}">
<xsl:if test="checked = 'true'">
<xsl:attribute name="checked">checked</xsl:attribute>
</xsl:if>
</input>
