You could build a variable with the sorted qa-elements and work on that,
using node-set()

(Seriously in a bigish (800 page) book we did for real, we did _not_ do
that and instead used trick xpaths wherever Norm (in his docbook
original) had used a simple <xsl:number/> to filter out what we
needed, but if doing it again I'd pre-process to get everything in the
right order first)
