DWARF 4 support, 0.148 plans

In the absence of any feedback, I've merged my debug_types branch into the
trunk, and added dwarf_offdie_types for use in iteration schemes. I've
finished the various DWARF 4 support bits in readelf, including using that
for .debug_types iteration.
It all remains untested. The .debug_types support at least should be
testable with GCC. The .debug_line/.debug_frame new formats are not
produced by anything extant, so those changes won't likely get tested.
I've asked Ulrich to be ready to cut the 0.148 release next weekend or so.
So we have the coming week to get in some good testing.
