It turns out that FOP does not support fo:table-and-caption, which is what
the XSL template for an HTML <table> element uses.  If the table is changed
to an <informaltable>, then it works in FOP, but you lose the caption.  I
filed a bug report on this and will fix it soon.  In the meantime, you could
convert the table to CALS markup.
