You should be getting indented TOC lines.  Are you using PassiveTeX to
convert the FO output to PDF?  Your use of the passivetex.extensions
parameter suggests so.  I think that is likely the problem. PassiveTeX is an
incomplete implementation of XSL-FO, and it is not under active development
to get it there.  You would get better results using another FO processor.
FOP is free, but the commercial processors do the best job.
