Here is my (validated to docbookx.dtd 4.1.2) test doc:

I convert it to fo with (v1.48 docbook xsl):
xalan test.dkb $XSLDIR/fo/docbook.xsl test.fo
and then:
pdfxmltex test.fo
Which seems to put me into an interactive mode. If I type ^D, then it
ends the process, with a message about the Fatal error.
For now I will follow the advice found in the message Bob linked to.
Since I can process the resulting test.fo file with fop, and get
a pdf with no errors, I am assuming that the problem is with my TeX
or PassiveTeX installation.
I installed PassiveTeX from SuSE Linux 7.3 rpm files. I can't find the
test you mention.
