First, try to use the latest stylesheet release (1.74b). It contains a
few fixes related to indexes.
Second, show an actual example that exhibits the behavior you observed.
Simple index terms like you show above are known to work, but there are
some contexts that have been the cause of trouble. But it's impossible to
know without seeing the context.
