Remove more shifts for sign/zero extension


The C standard says that right shifts of a signed type and a negative
value are implementation defined, ie. there is no guarantee that you
have an arithmetic right shift.  The standard also says that a left
shift of a signed type is only defined when the value is nonnegative
and the result is representable (ie. does not overflow).

So, sign extending with << followed by >> isn't guaranteed to work
(and will trigger a ubsan warning if you ever have a negative
result).  Most of the following patch addresses this problem.  The
exception is the arc-dis.c BITS macro which is OK with its current
usage provided chars are 8 bits, but I decided to fix it to prevent
future problems.
