ppc-opc.c formatting


	* ppc-opc.c: Formatting and comment fixes.  Move insert and
	extract functions earlier, deleting forward declarations.
	(insert_nbi, insert_raq, insert_rbx): Expand use of RT_MASK and
	RA_MASK.
